John Millman was born in Brisbane, Australia, as the youngest of a family of five children. He is the only brother of his four sisters.
During his youth, he attended the Brisbane Grammar School. Afterward, he attended the Anglican Church Grammar School.
At the age of 15, he made his ITF junior tournament debut in Darwin in 2004 where he reached the quarterfinals. Two years later, at the 2006 Australian Open, he made his junior Grand Slam debut.
In the same year, he won his first junior tournament at a tournament held in New Caledoni, then won his next two tournaments in a row held in Fiji and New Zealand respectively.
He competed in his final junior tournament at the 2007 Australian Open. The following year, he began to pursue professional tennis.
